Procedures and Forms
Registration for a District II soccer team is typically performed by one of the 17 leagues that make up District II. Most of these leagues have web sites that will direct you to the proper contacts and registration procedures. Please consult the D2 Leagues page or League Presidents page for the list of leagues and their web sites.
If you don't know which district you live in, we have a somewhat outdated Zip Code Lookup facility. Click on your zip code and league contact information will be displayed. It is highly recommended that you double check the league contact information in the zip code lookup facility with the league's web site on either the D2 Leagues page or League Presidents page. League web sites will usually have more current contact information.

CYSA Forms:
IMPORTANT: CYSA-N is very strict about what forms are allowed to be used in a given year. The most current information is typically on the CYSA web site.
